Parent/Guardian Application

Dear Parent/Guardian:

Congratulations! Your child has been nominated for the Minds Matter Program. Minds Matter connects driven and determined students from low-income families with the people, preparation, and possibilities to succeed in college, create their future, and change the world. Participation in Minds Matter can provide your child with enriching educational and social opportunities and help them achieve their dreams of college success.

Since inception in 1991, 100% of Minds Matter students have been accepted to a four-year college or university.

To be eligible for the program, a student must:

--Have a strong overall grade point average of 3.0 or higher (3.5 for Cleveland and
Portland chapters, 3.25 for Colorado chapter)
--Be committed to pursuing a college education
--Demonstrate strong academic potential, motivation, maturity, and responsibility
--Be active in extracurricular and/or community activities (part time jobs included)
--Show that he/she would benefit greatly from a summer academic experience
--Be committed to attending weekly Minds Matter sessions during the school year

If your child is accepted, Minds Matter will:

--Provide a comprehensive three-year program focused on college preparation.

--Improve his/her writing, public speaking and critical thinking skills through an extensive weekly tutoring/mentoring program

--Assign each student two (2) volunteer mentors who are working professionals with college degrees

--Guide students through the application process for summer academic programs and, eventually, for post-secondary education

--Raise funds to pay for all summer program costs

--Improve his/her standardized test scores through an extensive weekly test preparation workshop.

Please answer the following questions as part of your child’s application process.  Please note that providing incomplete or invalid income and asset information can serve as grounds for dismissal of a Minds Matter student.  We use this information for summer program, college and financial aid applications, as well as to determine their eligibility for the Minds Matter program.

Please Note: You must enter your child's First Name, Last Name, Birthdate and Chapter exactly as they do.  Please confirm all information with the student before submitting this form.
