What is an incident?
An incident is an undesired event, which resulted or could have resulted in harm, damage or loss.

An incident report MUST be completed if:
  1. An outside emergency service is contacted;
  2. An individual has received first aid or is taken to hospital, a doctor's surgery, emergency dental surgery, or seen by another medical professional;
  3. An injury results in a participant being unable to participate for 24 hours;
  4. An injury results in uncertainty of the outcome;
  5. There is an alleged illegal act, a participant sent home, serious leader dispute, damage or loss of property;
  6. When a near miss has occurred;
  7. Other situations will require judgment and consultation with Encounter Youth.

Your Team Leader may assist you in its completion and they are also required to sign off on the information provided below.

If not known to you, please state gender, approximate age and any other identifying details



Where did the incident occur? Please be specific, giving closest street address or any other landmarks


What did you observe, what was the situation, who was present, etc


What steps did you take to respond to the incident? What resources did you use? How much time did you spend responding? If an ambulance was called, how long did you wait and were they transported to hospital?
