And now, in small font, some words of great importance!
As a condition of volunteering, I give permission for 826LA to conduct background checks on me, which may include (but are not necessarily limited to) reviews of sex offender registries, child abuse, and criminal history records. If I no longer wish to submit to background checks, I agree to withdraw my application from consideration (or resign my volunteer position, if already accepted). I understand that false or misleading information on this application, or the discovery of inappropriate information through my background checks, may result in 826LA declining my volunteer application (or the termination of my volunteer position, if already accepted). I hereby release and agree to hold harmless from liability to 826LA, the employees and volunteers thereof, or any person or organization that may provide such information. I also understand that, regardless of previous appointments, 826LA is not obligated to appoint me to a volunteer position. If appointed, I understand that, prior to the expiration of my term, I am subject to suspension by the Staff and Board of Directors of 826LA for violation of 826LA policies.

Note: 826LA will not discriminate against any person based on the basis of race, creed, color, national origin, marital status, gender, sexual orientation, or disability.

Media Release
While you are engaged in programming at 826LA, you may appear in photographs, video, or radio footage that 826LA may wish to use in its promotional materials (which include the 826LA factbook and website) or on the radio. These materials help provide the public with information about programs at 826LA, allow students the opportunity to have their work aired on the radio, and are used to help 826LA raise money to support the writing center and its programs.

Please note that although permission for use of photographs, video, or radio footage of you is voluntary, once consent is provided by signing this form, it cannot be withdrawn. Also, 826LA is a non-profit organization and will not be able to provide monetary compensation. Please sign below to grant permission to use the material as follows:

I hereby grant permission to 826LA to use my photograph, video, and/or radio footage without payment or further consideration. I acknowledge the organization’s right to crop, treat, or edit the photograph, radio, or video footage at its discretion. I acknowledge that 826LA may choose not to use the photo or material at this time, but may do so at its own discretion at a later date. I acknowledge that 826LA will not provide monetary compensation for the photographs or material.
