Authorization to Release Information
I hereby authorize verification of all information in my volunteer application from all sources of employment, education, motor vehicle, financial history, criminal history, personal character, and worker's compensation records in accordance with ADA, labor and wage records, etc., or any part thereof, and authorize any duly authorized agent of IntelliCorp Records, Inc., to obtain, whether the said records are public or private, including those which may be deemed to be privileged or confidential in nature and I release all persons from liability on account of such disclosures. Information appearing on this Authorization will be used exclusively by IntelliCorp Records, Inc. for identification purposes and for the release of information that they will be relied upon in considering my application for a volunteer position. I agree to provide additional information that may be needed to process my volunteer application. I authorize without reservation, any party or agency contacted by IntelliCorp Records, Inc., to furnish the above-mentioned information. This authorization is valid during the course of my volunteer service to the extent permitted by law. 

I have the right to make a request to IntelliCorp Records, Inc., upon proper identification, to request the nature and substance of all information in its files on me at the time of my request, including sources of information, and the recipients of any reports on me which IntelliCorp Records, Inc., has previously furnished within the two year period preceding my request.

I understand and agree that any omission, false statement, misleading statement, or answer made by me on my application or any supplements to it and in any interviews will be sufficient grounds for rejection of a volunteer position and my release after acceptance in a volunteer position. 

I understand that Kids for Peace may rely on any part or all of this information in determining whether to extend to me a volunteer position. If adverse action is taken or if I am not offered a volunteer position based on the information obtained in the background check, I will be provided a copy of such information along with a summary of my rights under the Fair Credit Reporting Act. 

Disclosure Statement
As part of our hiring background and investigation, we may obtain consumer reports or prepare an investigative consumer report. The investigative consumer report may consist of contacting all listed prior employers to verify your employment history. It may also include, but not be limited to, credit information reports, criminal history reports and driving history records. Under the provisions of the Fair Credit Reporting Act (15 USC at 1681-1681U) as amended, before we can seek such reports, we must have your written permission to obtain the information. You have the right, upon written request, to a complete and accurate disclosure of the nature and scope of the investigation. You are also entitled to a copy of your rights under the Fair Credit Reporting Act.

As a Kids for Peace Chapter Leader:

• I understand this is an application for a volunteer position with Kids for Peace for which there is no monetary compensation. In the selection of volunteers, there shall be no discrimination against an otherwise qualified individual on the basis of race, color, ethnicity, sex, religion, creed, national origin, socioeconomic status, age, disability, marital status, veteran status or any other basis prohibited by federal, state, or local law.

• I give my permission to contact my educational, personal and employment references and/or conduct a background check. I further authorize these references to release to Kids for Peace  information that they may have about me.

• I certify that all information provided on this application is true and complete. I understand that falsification or significant omissions of any information may be considered justification for non-acceptance or dismissal if discovered at a later date. I also understand that acceptance for volunteer service is subject to verification of references and background check.

• I understand that my chapter is financially independent from Kids for Peace. Funding for chapter activities and projects is the responsibility of the chapter as outlined in the Kids for Peace Chapter Financial Policy.

• I understand that by default I will be placed on the Kids for Peace Chapter email distribution list as well as the Kids for Peace Global email distribution list. You may adjust the lists in which you are subscribed by scrolling to the bottom of our emails and clicking on "Update Profile".

As a Kids for Peace Chapter Leader, I agree to coordinate and secure all necessary release forms from participating youth and/or their guardians, including consent to be photographed and interviewed by the media. This permission includes without limitation the right for Kids for Peace, Inc. to use the child/children's name, likeness, image, voice, appearance and performance whether recorded or provided on or transferred to videotape, film, slides, photographs, audiotapes or other media, now known or later developed. This includes the right to edit, mix or duplicate, resize or crop and to use or re-use the photo and/or videotape in whole or part as Kids for Peace may elect for promotional campaigns, newspaper articles, brochures, grants, or any other program or product. Additionally, this includes the right to broadcast, exhibit, market and otherwise distribute the photos and/or videotape without any financial commitment or obligations to me as a result of this agreement. I agree to maintain updated copies of said forms on file and notify Kids for Peace and/or its representatives of any minor who is not to be interviewed or photographed when media is present. I expressly release and indemnify Kids for Peace, Inc. and its officers, employees, agents and designees from any and all claims knowns and unknown arising out of, or in any way connected with the above granted uses and representations. 

As a Kids for Peace Chapter Leader, I understand that my obligations are to:

1. Follow the principles of the Kids for Peace mission, vision and Peace Pledge.
2. Abide by the Kids for Peace policies and procedures for Chapter Leaders.
3. Stay in regular communication with headquarters.
4. Register for the Kids for Peace Chapter Leaders Resources page on the Kids for Peace website in order to gain access to Chapter Leader tools and resources.
5. Support, empower and protect all youth as they work toward creating a peaceful world.

I acknowledge full responsibility for the participating members of my Kids for Peace Chapter.

If/When my chapter meets on a third party organization's property, I recognize that the chapter operates under the responsibilities, policies and guidelines of the third party organization.

Finally, I release Kids for Peace, Inc. from any and all liabilities.

As a Kids for Peace School or Affiliate Chapter Leader, I understand that my obligations are to:

1. Follow the principles of the Kids for Peace mission, vision and Peace Pledge.
2. Abide by the Kids for Peace policies and procedures for Chapter Leaders.
3. Abide by any and all rules and regulations of the school or organization with which we are affiliated.
4. Collect and have on hand any necessary liability release forms or emergency contact information for each member of my chapter.
5. Stay in regular communication with headquarters.
6. Register for the Kids for Peace Chapter Leaders Resources page on the Kids for Peace website in order to gain access to Chapter Leader tools and resources.
7. Support, empower and protect all youth as they work toward creating a peaceful world.

I acknowledge full responsibility for the participating members of my Kids for Peace Chapter.

If/When my chapter meets on a third party organization's property, I recognize that the chapter operates under the responsibilities, policies and guidelines of the third party organization.

Finally, I release Kids for Peace, Inc. from any and all liabilities.

As a Kids for Peace USA Neighborhood Chapter Leader, I understand that my obligations are to:

1. Provide every member of my chapter this link to register annually as a member of Kids for Peace. It is my responsibility to ensure that members are registered prior to participating in chapter activities and that I have contact information for the guardians of chapter members on hand during my meetings. I understand this form protects me from liability and requires chapter member guardians to sign and assume all risk in chapter activity participation. As new members join my Kids for Peace Chapter, I will share this link with their guardians. I may be held responsible for chapter members whose guardians have not registered and signed this form. The membership form link will be emailed to me after I submit this chapter registration and my registration has been processed.
2. Record any income or expenses on a simple Financial Tracking Sheet that will be collected every July 15th.
3. Follow the principles of the Kids for Peace mission, vision and Peace Pledge.
4. Abide by the Kids for Peace policies and procedures for Chapter Leaders.
5. Stay in regular communication with headquarters.
6. Register for the Kids for Peace Chapter Leaders Resources page on the Kids for Peace website in order to gain access to Chapter Leader tools and resources.
7. Support, empower and protect all youth as they work toward creating a peaceful world.

If/When my chapter meets on a third party organization's property, I recognize that the chapter operates under the responsibilities, policies and guidelines of the third party organization.
