Please provide names and addresses of two people who know you well, who we could contact for a reference. Ideally one will know you in a professional capacity (eg a current or previous employer or volunteer supervisor, teacher, support worker or occupational therapist). The other could be a friend or neighbour.

Please note that a referee cannot be a relative. If you are having trouble finding references please discuss this further with the Volunteer Manager.

Criminal Record Declaration

This part of the form must be completed by all applicants. The information disclosed on this form will not be kept with your application form during the application process. The information will be treated as confidential and held securely.

 

We recognise the contribution that ex-offenders can make as volunteers and welcome applications from them. A person’s criminal record will not in itself prevent that person from being appointed to a role. Suitable applicants will not be refused roles because of offences which are not relevant to, and do not place them at or make them a risk in, the role for which they are applying.

Failure to disclose all unspent convictions will be dealt with using the volunteer problem solving policy and procedure and could result in a volunteer being asked to leave the organisation. 

This volunteer role is not exempt from the Rehabilitation of Offenders Act 1974. We only ask applicants to dis­close convictions which are not yet spent under the Rehabilitation of Offenders Act 1974.

All personal data that Bankside Open Spaces Trust may use in running our volunteer programme will be collected, processed, and held in accordance with the provisions of EU Regulation 2016/679 General Data Protection Regulation (“GDPR”) and your rights under the GDPR.

 

The information you give in this form will be used to assess your suitability as a potential volunteer and match you to the most appropriate roles and sessions available at the time of application. If you are successful in volunteering with BOST, we will use this information to contact you with information about the days, times and locations of your volunteering sessions. We will also, where you give consent, add you to the BOST emailing list for news about events and upcoming opportunities. We may share results of research that we carry out into the use of our services with third parties but this information will always be anonymised and will not contain your personal information. If you start volunteering with BOST we will keep this information for 3 years after you leave. If you don’t start volunteering we will destroy this information after 6 months. We take looking after your information very seriously. We've implemented appropriate physical, technical and organisational measures to protect the personal information we have under our control, both on and off-line, from improper access, use, alteration, destruction and loss.
