AUTHORIZATION TO RELEASE INFORMATION

I understand that the submission of an application for factoring with TBS Factoring Service, LLC (“TBS”) does not mean that TBS will factor or provide any financial services whatsoever. I further understand that approval to factor will come only after the Factoring and Security Agreement and all supporting forms have been signed and approved by TBS. I certify the accuracy of the information provided and understand that TBS will be relying on the accuracy of this information when evaluating the company’s application. By submitting this application either by mail, fax, or electronically, the company authorizes TBS to use any credit bureau or business to verify the validity and accuracy of all information contained herein. I consent to TBS filing of one or more Initial Financing Statements against me or the undersigned company in any or all Uniform Commercial Code jurisdictions, which reflect the collateral as “all assets.” DISCLOSURE OF RIGHT TO REQUEST SPECIFIC REASONS FOR CREDIT DENIAL GIVEN AT TIME OF APPLICATION (BUSINESS CREDIT) If your application for business credit is denied, you have the right to a written statement of the specific reasons for the denial. To obtain the statement, please contact Credit Disclosure Administrator, TBS Factoring Service, LLC,  PO Box 18109 Oklahoma City, OK 73154 Office (888) 707 5188 within 60 days from the date you are notified of the credit decision. We will send you a written statement of reasons for the denial of credit within 30 days of receiving your request for the statement. Notice: The Federal Equal Credit Opportunity Act prohibits creditors from discriminating against credit applicants on the basis of race, color, religion, national origin, sex, marital status, age (provided the applicant has the capacity to enter into a binding contract); because all or part of the applicant’s income derives from any public assistance program; or because the applicant has in good faith exercised any right under the Consumer Credit Protection Act.
