Dear participant,
In response to Coronavirus (COVID-19)
pandemic, Christ Community Health and the Augusta Fire/EMA are offering a Free COVID-19
screening to the Paine College employees on Thursday, June 25 from 9am to 12pm. The screening will take place at the
Information Center on the AHA property in a temporary outdoor screening
facility.
Testing, social distancing, and good hygiene
are our best defense against this virus.
Because this virus can spread from persons who don’t have symptoms, it is
important that all persons are offered the test. This is why you are being offered the test even if you don’t currently have
symptoms.
Testing is voluntary but we encourage all
residents to participate. The more persons who participate the better chance
all residents have in creating a safe environment for their neighbors. Unfortunately, our community has not yet seen
the worst of this virus. All test results will be communicated to
participants by the Health Department. By completing this form you are
consenting to be tested.
All residents, regardless of testing, will be
offered a free mask to wear at all times when in public spaces. We encourage all persons to wear a mask
according to Center for Disease Control (CDC) recommendations. This includes at all times when in public
spaces where social distancing cannot be strictly followed. (ex. Grocery Store, Drug Store, Elevator, Apartment
hallways and entrances).We will also be offering some education materials from
the CDC.
If you would like to participate in this
screening, please fill out the attached form and return to the Augusta Fire/EMA
by June 23 by 11am. Please bring a form of Identification (photo ID) to the screening.
