Volunteer Shift Change Form - Melbourne

This form is for Melbourne volunteers who are unable to attend their regular scheduled shift.

We would appreciate it if fortnightly and monthly volunteers to nominate a Make-up shift if they are able to do so.

Abbotsford and Moorabbin garden volunteers do not need to nominate a make-up shift.

Please let us know why you need to cancel your shift.

If you are unwell or injured you must be fully recovered and able to perform the tasks required of your volunteer role before you return. This may require a medical clearance.

If you have symptoms of COVID-19 please get a COVID-19 test and let FareShare know if you test positive. If you test positive to COVID-19 you are welcome to return to FareShare after 5 days if you no longer have any symptoms.
