Returns and Faulty Modem form - Delivery sites use only

This form to be used by delivery sites when they receive a modem back from a Skinny Jump customer, the purpose of this form is to bridge the communication between the delivery sites and Skinny support team who is resosible for delinking/resetting any Skinny Jump accounts. The information provided in this form is to notify the support team and be used to delink the customer account and also communicate with you and DIAA for further allocation of Jump modem.
