By clicking "Submit" I, and all individuals with me, understand that we are considered Potential Adopters. Potential Adopters recognize and understand the inherent risk associated with the interaction and care of animals, including but not limited to the possibility of any injury to person and/or property, including your existing pets, which Potential Adopters may suffer as a result of the unpredictable nature and/or provocation of animal(s). In consideration of this assumption of risk, Potential Adopters hereby knowingly and willingly waive any and all claims, damages or causes of action which Potential Adopters may have or incur against PAWS Chicago, its officers, directors, agents, affiliates, employees and/or volunteers as a result of the interaction and potential adoption of an animal from PAWS Chicago.
By clicking “Submit”, I hereby grant PAWS Chicago permission to use my likeness in a photograph in any and all of its publications, including website entries and marketing materials, without payment or any other consideration. I understand and agree that these materials will become the property of PAWS Chicago and will not be returned. I hereby irrevocably authorize PAWS Chicago to edit, alter, copy, exhibit, publish or distribute this photo for purposes of publicizing PAWS Chicago’s programs or for any other lawful purpose. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph. I hereby hold harmless and release and forever discharge PAWS Chicago from all claims, demands, and causes of action which I, representatives, executors, administrators, or any other persons acting on behalf of PAWS Chicago have or may have by reason of this authorization.
