Thank you for your interest in providing your students with an educational arts field trip this year! Please fill out this form as completely as possible to request a field trip. Trips are prioritized based on data collected below, please check your school percentages online HERE 

*Please note that the formula Act One uses to calculate the NCES Title I percentage is:

(FREE LUNCH ELIGIBLE + REDUCED-PRICE LUNCH ELIGIBLE) / TOTAL STUDENTS x 100 = Title I %


We will respond within 14 days regarding the status of your request. Your field trip is not confirmed until you receive a confirmation email from Act One.
