Green Social Prescribing Activity Directory

This form is for deliverers of activities in Camden and Islington parks that would welcome social prescribing referrals by health professionals to their activities.
 
Deliverers are asked to complete this form to give assurance to health professionals of the quality of the activity they offer. If you provide activities at multiple parks or locations, please submit a form response for each location.

Activity Location
We publish all  green social prescribing activities on our Green Social Prescribing directory, which includes a map of where the activity is located. So people can find you on the map, please provide the location of where your activity takes place. If you don't have a specific location, please just provide a general location so your information shows correctly on our map!





Instructor and Organisation Details










Welcome and Support

Newcomers could be welcomed in the following ways:

  • A warm and friendly welcome at the first point of meeting
  • Everyone introduces themselves at the start of a session
  • The instructor wears a name badge
  • The activity is explained clearly to the newcomer
  • Checking in with the newcomer during the session
  • At the end of the session the newcomer is asked if they enjoyed the activity.


Participants could be supported in the following ways:

  • Participants are sent a reminder message of the session a day or two beforehand
  • Everyone is greeted by name
  • The instructor aims to interact with all – especially quieter – participants before, during or after the session
  • There is an opportunity for participants to give feedback
  • There is an opportunity for participants to socialise with one another, for example, by visiting a café after the session.
