Fill out the following information and Just Roots will let your child’s primary care provider know that you are interested in enrolling in the program. The practice team will then check your child’s eligibility. If your child is eligible, they will reach out to you to complete the referral to the Farm to Family program. Due to the volume of interest and calls, practices will only reach out to families who are eligible. Please contact your practice if you need help getting food for your family.
