Family Counseling:
This specialized short-term program provides counseling related to conflict, concern, or questions about Sexual Orientation, Gender Identity, and Gender Expression, and is for parents, caregivers, and family members with LGBTQ+ children ages 5+.
All family members will need to complete a copy of this pre-intake form, as well as complete an intake session with an intake counselor at The Center.

After reviewing your responses to our questionnaire, it seems you are unfortunately not eligible for our counseling services at this time.
As a reminder, our eligibility requirements are as follows for individual counseling:
1. Community members who are uninsured or have public insurance
2. Community members with private insurance who have an annual income of less than $50,000
3. Community members who are not already engaged in a similar supportive mental health counseling service.

To register as a new patient at Callen-Lorde please email ALL of the following information to registration@callen-lorde.org:
1. A filled-out New Patient Registration and Consent form
2. A picture of your government-issued photo ID (examples include: NYC ID, Driver’s license, or Passport).
3 If insured: A photo of your insurance card (front and back).
Once your information is received, you can expect a phone call from (212) 271-7200 within the next 1-2 business days to complete the registration process.
If you are unable to register online due to technology or other issues, you can register in-person from 9:00am to 3:00pm, Monday through Friday. Please bring a government-issued photo id, and your insurance card (if insured).
