Media Release Agreement:

I, the Participant, grant Hillsong Africa Foundation & Hillsong Church (herein referred to as “the Producer”) the following:

• I authorise the Producer to record and include in media projects and related materials my name, likeness, image and/or voice (Photographs/Video/Recordings/Media)
• I agree that the Producer shall not pay me any fees or royalties in connection with my contribution to the Project including, without limitation, when the Project is used in any other form of media either by the Producer, or its affiliates. 
• The Producer may use and authorise others to use all or parts of the Media. 
• The Producer, its successors and assigns shall own all right, title and interest, including copyright, in and to the Project, including the Media, to be used and disposed of without limitation as Producer shall in its sole discretion determine.
• I agree that this consent is valid for 5 years from the date on this form, and I may be given the option to renew consent upon expiry. I also understand that I can contact Hillsong Africa Foundation to revoke this permission at any time.
• I agree that I will not assert or maintain against The Producer, their successors, assigns and licensees, any claim, action, suit or demand of any kind or nature whatsoever, including but not limited to those grounded upon invasion of privacy, rights of publicity or other civil rights, or for any reason in connection with your authorised use of my Photograph, Recording or Media in The Project.
• I agree that my first name can be used in promoting this material.
• I agree that this material can be published Hillsong Africa Foundation's website or on social media.
• I hereby certify and represent that I am over 18 years of age and have read the foregoing and fully understand the meaning and effect thereof.
