This application should be completed by NEW PPZ SCHOLARS ONLY!! Please upload the required documents (PDFs are preferred, however screenshots, images or scans will be accepted). YOU MUST HAVE COMPLETED AN ELIGIBILITY APPLICATION AND RECEIVED A PONTIAC PROMISE ZONE ACCEPTANCE LETTER FIRST!


Submitting incomplete or inaccurate documents will result in a delayed processing of your application! If you have any trouble submitting your documents with this form, please contact us at scholarship@pontiacpromisezone.org or 248-214-0456.

Financial Aid Award Letter
Financial Aid Award Letter: We need to know that you have applied for FAFSA (Free Application for Federal Student Aid) or have received a financial award letter from your college in order for you to get our scholarship. Upload a screenshot or image of your financial aid letter. This applies even if you do not receive aid from FAFSA.
Here is an example of what an award letter looks like. (NOTE: DACA students will submit their signed PPZ FAFSA Waiver here instead.)
Itemized Tuition Bill
Tuition Bill:   An itemized tuition bill should include the following:

1. Number of credits being taken for the semester
2. All tuition related charges for the semester (NOT ESTIMATED COSTS)
3. All financial aid awards being applied for the semester (NOT ESTIMATED AWARDS)
4. Your remaining semester balance 


(NOTE: If you are unsure of how to access your itemized tuition bill, please reach out to your school/program’s financial aid office!)
