If your Healer's Touch Sculpture or DAISY Honoree pin has been lost or damaged, you may request a replacement below. Please note that there is a $60.00 shipping and handling fee for a replacement sculpture and a $5 donation request for replacement pins.

Honoree pins are for nurses who received The DAISY Award (this includes the sculpture, Honoree pin, gift bag, and celebration). The nominee pin is for any nurse who was nominated for the DAISY Award and should have received the pin and a copy of the nomination.

How many Nominee Pins do you need replaced? Please note, we will contact your organization for verification.

How many Honoree Pins do you need replaced? Please note, we will contact your organization for verification if you are not registered with us for each pin. You may also email an image of your multiple sculptures to ErinAscher@daisyfoundation.org

For quality control of our product, can you please let us know why you need multiple pins? Example: Did they break/ get lost/ no longer looks nice?

Please Note: Replacement pins ship once a week via USPS. It may take 3-4 weeks to receive your replacement, depending on your location. If you have not received your replacement after 4 weeks, please contact ErinAscher@DAISYFoundation.org directly for a status update/ tracking information.
