This form is for brands who are looking to use the Land to Market seal and verified regenerative claims on product packaging.

Please complete the following information for each product you are requesting verification for. To add multiple products click "add another response".

			If you are using multiple Land to Market materials in a product, please list the percentage of each material separated by a comma.		Please upload the product packaging art file (food products) or hang tag/ website copy (fashion products) for review and approval. If submitting multiple please upload as one file.

If you have a large number of products to submit you can upload a CSV/Excel file with Product Name, UPC/SKU, Expected Launch Date, % of Land to Market material in one product unit and the product Art File.

The respective entities have knowledge of all the matters stated in this affidavit, and true and correct signatures of officers with authority to sign on behalf of said entities appear below."

2. "We do hereby swear and affirm that the Land to Market verified product(s) listed remained segregated from NON-Land to Market Verified livestock-derived materials throughout our ownership of the product(s) (including any storage, transportation, processing, and handling by subcontractors).”
