PLEASE REMEMBER:
NO nuts or nut products of any kind. They are not allowed on school premises as some campers and staff have severe nut allergies.
Provide camp staff with specific information you feel is required pertaining to your child. Please let your camp staff aware if your child takes medication.
It is also ESSENTIAL that an authorized person sign the camper into the camp room each day, starting on Tuesday, at 9:00am and signs out from the camp room each afternoon at 4:30pm. If there is a change in pick up or drop off, please notify the school ASAP. Unless you have prearranged for extended care, extended care charges will be applied if you child arrives early or is picked up after the allotted time.
You will be required to provide us with a list of authorized people to drop off or pick up your child(ren) upon registration. All authorized people on your list must show a VAILD PHOTO ID each time when dropping off or picking up your camper(s).
Please note that campers will NOT be released from the program each day without a signature and valid photo ID from their authorized pick up personnel. Campers are not to leave school premises during the day unless accompanied by a camp staff member. This is for their safety! Any students who leave the school without authorization will not be allowed to return to camp for the rest of the week.

The 2024 Parent's Guide to Merriam Camps is available on our website. Please ensure to review it. It is understood that by completing this waiver, the parent/guardian has reviewed the document and is in agreement of the terms and conditions stated in the Parent's Summer Camp Guide 2024.
CAMPER DROP-OFF/PICK-UP:
*Please note: Campers will not be allowed in or out of camp each day without the signature and valid photo ID from their authorized pick-up personnel for safety reasons. It is essential the authorized person(s) sign their camper in and out each day. If there is to be a change in the authorized person(s), please notify the school as soon as possible. Please be aware that the names of authorized pick up people must match the valid photo ID given when the child is picked up.
VIDEOTAPING WAIVER and RELEASE:
The Camper consents to being photographed, interviewed, and/or videotaped by representatives of Merriam Music Inc. (Located at 2359 Bristol Circle Oakville, Ontario.)
Any information, images, or video obtained from those activities may be reproduced by Merriam Music Inc. for use in advertising, publicity or educational activities including, but not limited to, web sites, publications, videos, as a digital video file, print and television for promotional purposes. I understand that my name or my children(s) name(s) will not be published at any time. I hereby waive any claims I may have, and release Merriam Music Inc. and its representatives from liability of claims arising out of such activities. Finally, I do hereby declare that Merriam Music Inc. and its employees and representatives are in no way responsible and/or owing financially or otherwise for the taking of such video and photographs, or the use thereof.
WAIVER:
I permit Merriam Music Inc. to provide/obtain medical care as they see fit in the event of injury or otherwise and agree that I will be responsible for any expenses incurred. I am aware the Merriam Music Inc. staff are instructed not to administer medication, such as Asthma inhalers or Epi-Pens, but rather guide our child to use it as they see fit. The Merriam Music Inc. staff will make every effort to keep all campers safe and cared for. In case of any accident resulting in loss, damage, injury or expense, we agree to release Merriam Music Inc. from liability during our presence on premises owned or occupied by Merriam Music Inc. during the camp. It is understood that campers may be accompanied by their camp staff across the street to Thorne Lodge School to play in the yard, and I assume all risks that are involved.
