FAIR CREDIT REPORTING ACT
DISCLOSURE AND CONSENT FORM
You
are notified that, in connection with originating, processing, underwriting,
reviewing, servicing, and otherwise managing the loan for which you are
applying, a consumer report and/or investigative report which may contain
public record information may be requested and/or made on you. These reports
may include consumer credit, criminal records, driving records, education
history, prior employment verification, and other information for the purpose
of considering your creditworthiness, your eligibility for a loan, your ability
to repay any such loan, with Solar and Energy Loan Fund of St. Lucie County,
Inc. (“SELF”) and The Clean Energy Loan Fund of Texas, Inc. (“TxCEF”).
Information may also be requested from various federal, state, local, and/or
other agencies. Information obtained from these sources may be used to
disqualify you based on the contents of the reports.
You further understand
and acknowledge that TxCEF and/or SELF will originate, process, underwrite,
review, service, and otherwise manage the loan for which you are applying, if
you are approved. You explicitly authorize SELF and TxCEF to share the
information contained in your loan application for the purposes of evaluating
it for eligibility for a loan and as otherwise permitted by applicable laws,
including state and federal privacy and data security laws. Additionally, you understand and acknowledge
that SELF and TxCEF only collect social security numbers solely to obtain your
credit report to evaluate your financial standing and eligibility for a loan.
You
will be provided the name and address of the consumer reporting agency to which
the request for information is being made. SELF’s prime credit bureau resource
is Equifax, but SELF is not limited to only Equifax. You will have the right to
a complete disclosure of the nature and scope of the investigation. A written
summary of your rights under the Fair Credit Reporting Act are attached.
