Our company takes employee complaints of discrimination, harassment, and unethical or unfair conduct and labor practices as serious matters. So that we may thoroughly investigate your concern, please fill out this form as completely as possible. After our investigation into your complaint you will be notified of the company's intended action. If you have any questions about the process, please include them at the end of this form and we'll do our best to answer them. Thank you.

Confidentiality - we cannot guarantee confidentiality because once a complaint is brought to our attention, we will investigate. However, the investigation team will take reasonable steps to preserve, to the best of their ability, the confidentiality of the circumstance and their findings.

Retaliation - RMYC prohibits retaliation against an employee for filing a complaint under this policy or for assisting in a complaint investigation.
