We hope this offers you a meaningful opportunity to think about your organization and your own personal leadership. Because all too many programs only center executive directors and real change needs full team support and buy-in, we also welcome and encourage you to include your teams in this process, inviting in their perspectives and feedback.
Sections 1–4 are to be filled out by the executive director with team support and input. Section 5, on Page 4, features a downloadable Microsoft Word document for a board member to complete and return to you, which should then be uploaded as part of this form. Additional board and team members are invited to give input to this document, should you wish.

We encourage you to share the completed Reflection Form, including the uploaded Word document from a board member, with us whenever you are ready. The last moment to share this information with us is February 19 at 11:59 p.m. ET. Please note: Before continuing further, please know The Project pilot is only open to organizations based in the United States (including Puerto Rico) and Canada.
