Have you applied for a GusNIP Produce Prescription grant before?

Has your organization operated a produce prescription program before? This can be GusNIP funded or independently funded.

Does your organization have experience or have you identified an external evaluator with experience in program evaluation comparable to the requirements of a GusNIP grant? Typically, the sample size for a GusNIP PPR award ranges from 100 to 130 participants completing pre- and post-surveys on a yearly basis. Grantees must also work with an Institutional Review Board (IRB) to get their study approved and collect data on healthcare use and associated costs.

Did you receive a Capacity Building and Innovation Fund (CBIF) award in 2023 to support a future GusNIP Application?
