This online form will allow you to submit your AC Boost Annual Occupancy Certification materials online, including a self-certification of owner occupancy and supporting documents. Once you submit your form and documents, they will transfer into our client management system. Please be assured your information is treated as strictly confidential and is never shared beyond Hello Housing and the County of Alameda.

Copy of most recent homeowner's insurance policy declarations page (all pages) showing the insurance coverage, additional mortgagees/additional insured, address of your home and the name of at least one of the owners on title.

Copy of your mortgage statement dated within the last two months.

Self-Certification and Submit
