Donor Recognition Form

Thank you for your generous gift to the Center for Food, Youth & Community!

We will recognize every gift to this capital campaign on our website. If you have made a gift between $1,000 and $9,999, your contribution will be recognized with a personalized tile on the Community Donor wall at the new building. Please complete this form to indicate the text you would like on your tile. If your gift was $10,000 or more, we will be in touch with you about recognition and naming opportunities.

Your donor tile will include the exact message you submit. Be careful of spelling errors or typos. If you do not wish to be recognized in the Center, you may note that on the gift amount question.
