By submitting this form, I hereby give permission for images of me and my child(ren), including, but not limited to, photographs and videos submitted, to be used or published, in any medium with Blue Star Families, including partners and their affiliates, now or hereafter known, and for any purpose whatsoever, including (but not by way of limitation) illustration, promotion, advertising, and trade, and;

I hereby release and discharge the photographer and Blue Star Families, Inc. including partners and their affiliates, from any and all claims and demands ensuing from or in connection with the use of the photographs, including any and all claims for libel and invasion of privacy.

I understand that I will not be entitled to compensation for the use of these images. This authorization and release shall inure to the benefit of the legal representatives, licensees, and assigns of the photographer, as well as Blue Star Families, Inc, including partners and their affiliates.

I have read the foregoing and fully understand the contents hereof. I represent that I am the parent/guardian of the above-named child or children, and I hereby consent to the foregoing on his/her behalf.
