Hello San Leandro Applicant!

 

For applicants of the City of San Leandro's Below Market Rate ownership program, we have created this online form which allows each household to submit their application and upload supporting documents, securely, online. Once you submit your form and documents, they will transfer into our client management system. Please be assured your information is treated as strictly confidential and is never shared beyond Rise Housing and the City of San Leandro 

 

All applications will be reviewed on a First Complete First Served basis. Applications will not be accepted in person, with no exceptions.

Document Checklist

Please review the following to ensure you successfully submit a complete application. The items below are required for all adult household members aged 18 and over.

A. Proof of Identity

  • CA Driver's License/CA Identification Card/Municipal ID
  • US Passport
  • Social Security Card

B. Documentation of Income

  • Employment information, including paystubs for the past two (2) months
  • If self-employed, YTD Profit & Loss Statement
  • Gross annual income for each additional income source such as SSI, Pension/Annuity, Gift Income , child/spousal support etc. for the past two (2) months
  • If a household member is NOT earning income, please include a Zero-Income Affidavit (If needed, please request from Rise Housing)

C. Tax Documents

  • Two (2) most recent tax years of Federal 1040 Tax Returns
  • 4506-t Verification of Non-Filing Form for past two (2) years
  • Two (2) most recent tax years W-2s/1099s
D. Financial Accounts
  • The most recent two (2) consecutive statements for all financial accounts including bank, retirement, stocks/mutual funds, CDs etc.
  • Documentation for any unlabeled deposits on your bank account for over $500
E. Prequalification letter from a lender for, at minimum, the purchase price of the home you are purchasing.

If you are not able to finish filling out the recertification in one sitting, please follow these steps:

  3. Afterwards you'll receive an email with a link that can be used later to open the recertification and continue working.
  4. When you return to complete the form, you will need to provide the same email and password.

By providing this information, you are helping us better understand the reach and impact of our programs in the communities we serve, which in turn leads to more effective affordable housing solutions.

Household Economic Profile

Tax Information
If you or any household member cannot locate your returns, please contact the IRS to request tax transcripts at (800) 829–1040 or online at  https://www.irs.gov/individuals/get-transcript. If you or any household member were NOT required to file taxes for the last tax year, please provide a “Verification of Non-Filing”, which you can request by submitting a Form 4506-T to the IRS. This form is available online at http://www.irs.gov/pub/irs-pdf/f4506t.pdf or you may request it by contacting the IRS at (800) 829-1040.

The information you provide on this page helps determine your eligibility for the financial requirements of the City of San LeandroBMR Program.
