This event is open to all participants to allow access to our communities with an emphasis on our BIPOC, LGBTQ+ Community. 

We are creating programs and spaces where we can ensure people are feeling welcomed and represented. Our goal is focusing on historically excluded communities. We do this to ensure we are thinking about communities that traditionally would not attend or participate in our events. This is important to us as we are a world where the global majority population has a platform for creative expression.

I recognize that risk of illness and/or injury may be inherent in any dance or exercise class; however, the above identified individual is participating with the express agreement of, and understanding that Seattle Theatre Group, and its directors, officers, employees, partners and agents are hereby released from any and all claims, costs, liabilities, expenses, or judgments associated with this event. I hereby execute and deliver this Waiver and Release form to induce SEATTLE THEATRE GROUP to permit the above identified student to participate in this event. 

 

I hereby understand that SEATTLE THEATRE GROUP (STG) may take photographs and/or video of the participants enrolled on this form, and I authorize STG to use these photographs and video for archival and marketing purposes via brochures, website, and social media.



I hereby grant the SEATTLE THEATRE GROUP’s staff permission to administer first aid and/or call 911 in the case of a medical emergency while I attend classes. I realize and understand that SEATTLE THEATRE GROUP’s staff will attempt to notify emergency contacts in case of an emergency. 


In Witness Whereof, I have executed this waiver and release. 




Photo Release
I hereby understand that Seattle Theatre Group may take photographs and/or video of the participant indicated in this form, and I authorize Seattle Theatre Group to use these photographs and video for archival and publicity purposes.
