Thank you for volunteering with SV[e]F to share your knowledge and skill set with our students! 

Our STEM Career Chats take place throughout the summer, wherein our STEM Presenters (you!) share about their journey and experience in the STEM industries. Presentations last about 30 minutes and will include a Q&A where students can learn more about you. Prior to each presentation, volunteers will attend a brief Volunteer Orientation with SV[e]F Staff and other volunteers. 

Please complete this short form and sign our "Code of Conduct" Policy and image release.

By completing this form, you hereby release, indemnify and hold harmless SV[e]F, its offers, directors and employees, and the organizers, sponsors, and supervisors of all SV[e]F activities from any and all liability in connection with any injury you may sustain (including any injury caused by negligence) in conjunction with volunteering with SV[e]F. To ensure student safety, SV[e]F will conduct a background check before confirming your volunteer placement.

Volunteering takes place Monday through Friday between June 17th  through July 26th, with STEM Career Chats taking place on the Tuesday, Wednesday, and Thursday of each week. The presentations are generally held sometime between 9:30 am and 11:30 am. Please select the weeks you would be available to volunteer.


Volunteer Code of Conduct
Silicon Valley Education Foundation (SV[e]F) serves youth. SV[e]F volunteers teach valuable lessons with each word and especially with their conduct. Adult misconduct with youth carries serious consequences. SV[e]F cares that its volunteers have healthy relationships with the youth they serve, and agree to the following set of standards:
  1. I agree to conduct myself in a professional, respectful manner at all times. I understand that the volunteers are role models for students, and will conduct myself accordingly.
  2. I will maintain appropriate physical boundaries at all times.
  3. I understand that a school has a strong commitment to diversity and inclusion of all students. I will uphold those policies and values while volunteering. I will not share my personal or political views with students while volunteering. If I have any questions about this, I will ask teacher or SV[e]F staff.
  4. I will not engage in private communications with minors to include communications via text messaging, e-mail, phone, internet chat, on-line games, or other forms of social media
  5. I will share with teachers and/or school administrators any concerns that I may have related to student welfare and/or safety.
  6. I will wear or show volunteer identification whenever I am volunteering in a school. I will use only adult bathroom facilities.
  7. I agree never to be alone with individual students (including in an online environment) who are not under the supervision of teachers or school authorities.
  8. I will not possess weapons of any type while on school premises.
  9. I will not possess or use tobacco or any nicotine-related products and will not possess or be under the influence of alcohol, marijuana, or illegal drugs on school property or during school events off premises.
  10. I understand that in the course of volunteering, I may learn private or sensitive information about students, families, or staff. I agree to maintain confidentiality outside of school. I will not share information that I learn about students or staff with anyone except to carry out my volunteer duties and as outlined in this code of conduct.
The above applies regardless of whether the volunteer work is in-person or in a virtual environment, or a mixture of the two. These are examples only, not a comprehensive list. Other actions not listed could result in suspension or dismissal as a volunteer. SV[e]F takes all complaints of misconduct seriously. Credible allegations of misconduct will be promptly investigated by the appropriate authorities. 

Agreement: I agree to follow this code of conduct, the general standards of professional conduct, and maintain confidentiality. I understand that failure to comply is grounds for immediate suspension or permanent revocation of my volunteer privileges. 

 

By typing my name below, I confirm that I have read and understood this Code of Conduct and that as a volunteer for Silicon Valley Education Foundation I agree to follow these standards. I understand that any action inconsistent with this Code of Conduct or failure to take action mandated by this Code of Conduct may result in my dismissal as a volunteer.



Publicity release for likeness used by SV[e]F or corporate partner materials

I hereby grant SV[e]F, and our corporate and foundation partners, permission to use my name, picture, image, likeness, voice, biographical information and portrait and any statements, quotes or testimonials provided by me (collectively, “My Likeness”) in connection with SV[e]F, or corporate/foundation partner, events and services, and materials and activities that promote SV[e]F, or corporate/foundation partner, products or services (collectively, “SV[e]F Materials”). This includes the worldwide right to use, copy, distribute, exhibit, translate, broadcast, transmit, perform, publish, display, alter and create derivative works from the SV[e]F Materials containing My Likeness in any manner and in any media now or hereafter devised. SV[e]F shall have the right to edit, rearrange, vary, change, alter, modify, and delete My Likeness in the SV[e]F Materials, so long as the use of My Likeness is not misleading, disparaging or defamatory.


SV[e]F is under no obligation to make use of or to provide compensation for any of the rights or permissions granted by this Release. SV[e]F shall be the exclusive owner of all right, title and interest, including copyright, in the SV[e]F Materials. I hereby waive the benefits of any provision of law known as “droit moral” and/or “moral rights” or any similar law in any jurisdiction.


I am over the age of eighteen and legally competent to sign this Release. I understand that the terms herein are contractual and not a mere recital and that this instrument is legally binding. I have signed this document of my own free act.


This agreement contains the parties’ entire understanding relative to its subject matter. This Release is intended to be as broad and inclusive as permitted by applicable law. In the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such a clause or provision shall not otherwise affect the remaining clauses and provisions of the Release, which shall continue to be enforceable.
