Application Step 2 - Complete the GSA Insurance Form to Receive Certificates

► Combined Liability (Public and Products) Insurance is available for organisations with a gross annual income under $1m

► Complete your insurance application through the GSA website linked from this form

► Insurance applications can take 2-4 weeks to be approved

► Insurance certificates valid to 31 October 2024 will be sent to your nominated email address

► Insurance certificates valid to 31 October 2025 will be sent by email in late October 2024

INCLUDED Volunteer Workers' Insurance
As part of your Regional Arts Victoria Small Creative Organisation Membership you are eligible to apply for Volunteer Workers cover under RAV's master insurance policy. You will need to go to step 2 at the end of this form and complete your insurance details in GSA Insurance Broker's website to receive your certificate of currency.

Personal Accident Insurance for Volunteer Workers protects volunteers and members of your organisation from expenses incurred due to injury sustained while engaged in official unpaid voluntary activities (this could be a work experience student or someone volunteering to attend a meeting). Visit this page to learn more.
INCLUDED Association Liability and Volunteer Workers' Insurance
As part of your Regional Arts Victoria Small Creative Organisation Membership you are eligible to apply for Association Liability and Volunteer Workers cover under RAV's master insurance policy. You will need to go to step 2 at the end of this form and complete your insurance details in GSA Insurance Broker's website to receive your certificate of currency.

Association Liability cover is essential to protect your organisations directors, office bearers and committee members in the incident of a claim for a wrongful act. Visit this page to learn more.

OPTIONAL Combined Liability Insurance (including Public Liability)

We offer Small Creative Organisation Members access to optional Combined Liability Insurance (including public, product and advertising liability cover) for an additional fee, calculated based on gross annual income.

Insurance Fees 
Insurance fees are calculated based on your Gross Annual Income (GAI). 
  • $275 (GAI Up to $5,000)
  • $440 (GAI $5,001 to $10,000)
  • $660 (GAI $10,001 to $20,000)
  • $990 (GAI $20,001 to $50,000)
  • $1,380 (GAI $50,001 to $100,000)
  • $2,300 (GAI $100,001 to $200,000)
  • $4,025 (GAI $200,001 to $500,000)
  • $4,200 (GAI $500,001 to $1M)
Organisations with Gross Annual Income over $1M are ineligible for Combined Liability Insurance.
