This is our primary means to communicate news and updates about the CSA to members, and everyone is added at registration. If you choose, you are welcome to un-subscribe to our emails.





We encourage all members to subscribe to text alerts as this is a useful way to communicate reminders and last minute changes to pick-ups such as bad storms! You can opt out at any time by responding STOP
Member Info
We ask the following to make sure we meet everyone's needs — inclusive of race, ethnicity, age, gender, sexual orientation, and CSA experience. Every question in this section is optional, but your willingness to share helps us evaluate our program and access funding resources.











Delivery
You may request delivery of your CSA share if one of the following circumstances describe your situation AND you in Massachusetts, within 5 miles of the farm.
    • You have limited mobility that prevents you from coming in person 
    • You have high risk conditions, or are otherwise vulnerable due to Covid-19
    • You live at Leyden Woods, Oak Courts, or Elm Terrace

Payment Options

The Three Season CSA costs $400 for seven months of produce November-May. You have the option to pay in full now, or pay a $100 deposit followed by three monthly payments of $100 from November-January.

If you would like to pay with SNAP/HIP, please submit an AutoSNAP form (attached on the following page), and $57/month from November-May will be automatically withdrawn from your SNAP account.



You are permitting Just Roots to deduct $57 per month from your SNAP account from November through May. Please follow the instructions to sign up and then sign this form before submitting!



Paying with Auto-Snap
To use SNAP to pay for a CSA share, we use something called an "Auto-SNAP" form! It's a form that lets Just Roots automatically receive your payments on a set monthly schedule! It saves time and energy for you and for us! See the steps below to complete the form.

If you have any questions about Auto-SNAP or the Health Incentives Program (HIP), or you don't think you have enough SNAP money to cover the cost of the share, give us a call!

  2. Print and fill out the form or use a photo editor on your computer like Adobe or Preview
  3. After your form is filled and signed, take a clear photo and upload it below.

Media Release
Media release conditions: I hereby grant Just Roots permission to use my likeness in a photograph, video, or other digital media in any and all of its publications, without compensation or other consideration. 

I understand and agree that all media in which I appear as a part of my CSA membership are the property of Just Roots.

I hereby irrevocably authorize Just Roots to edit, alter, copy, exhibit, publish, and distribute media in which I appear in lawful ways. I waive any right to inspect or approve this media. 

I hereby hold harmless, release, and forever discharge Just Roots from all claims, demands, and causes of action which I, or any other persons acting on my behalf have or may have by reason of this authorization.

Member Commitment
By signing up for a CSA share with Just Roots, I am committing to a season of fresh, local, naturally grown vegetables. I understand that the CSA model is one of risk and reward. In most seasons, we share in the great bounty! In a harder season (drought/floods), I may receive less than I hoped for. I trust that our farmers are doing all they can to provide me with a bountiful share, and by participating in this farm share, I am pledging my support to my farmers and farm. Together we make a strong farm community!

Calculated field. If $0, iATS will not set up a recurring payment schedule. 

Static field that can be updated by JR. It's used by iATS for the scheduled payment start date if the person is paying in multiple installments rather than all at once. Changing this will change the scheduled payment start date for all new submissions. The scheduled payment is handles separately than the one-time (due now) payment so the start date should be for when the scheduled payment starts.
