SUBMISSION INSTRUCTIONS: 

In addition to the application submitted in eGrants, applicants are required to submit the following additional documents by the application submission deadline. Based on the documents you will  submit via the instructions in the Notice, change the status in eGrants from the default “Not Sent” to the applicable status “Sent,” “Not Applicable,” or “Already on File at AmeriCorps.”


Additional Documents must be submitted via this form. 


Individually saved files that must be clearly labeled in the required naming convention which include the legal applicant nameand application ID number within the file name and heading of each document.


All documents must be submitted to the GOSV regardless of prior submission via this form by the deadline(s) specified in the Notice of Funding Opportunity.

Evidence Documents

Labor Union Concurrence


    1. If a program applicant:

      1. Proposes to serve as the placement site for AmeriCorps members; and

      2. Has employees engaged in the same or substantially similar work as that proposed to be carried out by AmeriCorps members; and

      3. Those employees are represented by a local labor organization, then the application must include the written concurrence of the local labor organization representing those employees. Written concurrence can be in the form of a letter or e-mail from the local union leadership.

    2. If a program applicant:

      1. Proposes to place AmeriCorps members at sites where they will be engaged in the same or substantially similar work as employees represented by a local labor organization, then the applicant must submit a written description of how it will ensure that:

        1. They will not place AmeriCorps members in positions that were recently occupied by paid staff.

        2. They will not place any AmeriCorps member into a position for which a recently resigned or discharged employee has recall rights as a result of a collective bargaining agreement, from which a recently resigned or discharged employee was removed as a result of a reduction in force, or from which a recently resigned/discharged employee is on leave or strike.


For the purposes of this section, “program applicant” includes any applicant to AmeriCorps or a State Commission, as well as any entity applying for assistance or approved national service positions through an AmeriCorps grantee or subgrantee.

Federally Approved Indirect Cost Rate

Federally approved indirect cost rate agreement, if used to claim indirect/administrative costs. Applicants applying that include a state or federally-approved indirect cost rate amount in their budget or using a de minimis rate must enter the current approved indirect cost rate agreement information into eGrants at the same time they submit their application, and must submit via email the approved indirect cost rate agreement. See Indirect Cost Rate User Instructions.

Federal Debt Deliquency and debarment certification

FINANCIAL DOCUMENT SUBMISSION

MOST RECENT AUDIT OR INDEPENDENT FINANCIAL MANAGEMENT LETTER

Most recent audit or independent financial statement with management letter (single audit if organization spends $750,000 or more of federal assistance).

Non-Profits only:  A copy of the Form 990 that was most recently filed as of the date of the submission.

Operational and Financial Management Survey

Operational and Financial Management Survey. Complete survey here. Survey responses are sent directly to AmeriCorps and the MD GOSV does not review responses. The person completing the survey will receive a confirmation email that the survey was submitted. Submit the confirmation email to the MD GOSV as part of the additional documents.

Located on the MD GOSV website. This document must be submitted as an excel document.

Evaluation

Evaluation Plan, if required. Evaluation plan template on the MD GOSV’s website must be used and submitted as a Microsoft Word document. Evaluation plans are not scored and will not be reviewed until after funding decisions are made. Applicants who have received three or more years of AmeriCorps funding for the same project being proposed, must submit an evaluation plan via email to joy.mcginnis@maryland.gov by the application deadline.

Evaluation report, if required (recompete only). Applicants that have received six or more years of AmeriCorps funding for the same project being proposed should provide an evaluation report. Please submit in Microsoft Word. The evaluation report should include a title page with:
  1. the AmeriCorps grant number for the project that was evaluated,
  2. the name of the project,
  3. the date of completion of the report,
  4. the evaluator name(s)
  5. evaluator organization name and,
  6. evaluator type (internal or external)
Applicants who have received six or more years of formula funding for the same project being proposed are highly encouraged to submit an evaluation report with their application. If this is not feasible, reach out to the MD GOSV at joy.mcginnis@maryland.gov for an alternative due date. If any of this required information is missing, the applicant may not receive credit for meeting their evaluation requirements.
