I, the undersigned, understand that:

this application does not mean an immediate acceptance to the Spiritual Direction Program, but a request for consideration to be a participant.
there MAY be a personal interview required for entrance to the program and this interview may be done via phone.
there is an ongoing discernment for the suitability of each accepted candidate throughout the duration of the program and that each candidate is under probationary status during the program.
each accepted candidate is expected to abide by the code of conduct during the length of the program and upon completion of it.
these terms and conditions are subject to change.

Code of Conduct:

I, the undersigned, understand that if accepted into the Avila Institute’s Spiritual Direction Program, I am expected to:

complete the required pre-requisite distance learning courses prior to in-house sessions with Heart of Christ.
acquire a spiritual director/mentor during the program.
be faithful to the teachings of the Magisterium of the Roman Catholic Church AND BE IN GOOD STANDING
A LETTER OF GOOD STANDING FROM YOUR PASTOR WILL BE REQUIRED AS PART OF THE APPLICATION PROCESS
understand that this code of conduct is subject to change.

Video and Photography Release:

I, the undersigned, hereby grant the Avila Foundation the irrevocable right and permission to use photographs and/or video recordings of me on Avila Foundation and other websites and in publications, promotional flyers, educational materials, derivative works, or for any other similar purpose without compensation to me.

I understand and agree that such photographs and/or video recordings of me may be placed on the Internet. I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany the photographs and/or video recordings of me. I waive the right to approve the final product. I agree that all such portraits, pictures, photographs, video and audio recordings, and any reproductions thereof, and all plates, negatives, recording tape and digital files are and shall remain the property of the Avila Foundation.

I hereby release, acquit and forever discharge the Avila Foundation, its current and former trustees, agents, officers and employees from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the use or distribution of said photographs and/or video recordings, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation.
